The Integrated Care Clinic at Orion Medical is a patient-centered clinic within our practice, focused on providing specialized care for patients with heart failure. Working alongside your primary cardiologist, our team—including a heart failure–trained Nurse Practitioner, Registered Nurse, and Medical Assistant—offers personalized support to improve outcomes, reduce ospital visits, and help you reach your wellness goals. Who Qualifies for an Integrated Care Clinic?
Enrollment is currently available for patients who have a diagnosis of heart failure and are currently seeing an Orion Medical cardiologist.
